ICWS’ Al-Andalus Sunday program will be provided in person.

What is offered

Arabic: Grammar, Reading and Speech. Taking your child from a beginner reader to a speedy reader, who can also converse with ease.

Quran: Focusing on pronunciation via talqeen for the younger and ahkam for the older with perfection and speed.

Islamic Studies: Will be diving with your child into learning with hands-on project topics focused on raising a better Muslim.

General Waiver and Statement of Consent

I fully testify that the information on this form is correct and filled according to the best of my knowledge. As a parent/guardian, I pledge and promise that I will do my utmost to fulfill my responsibilities of supervising educating and supporting the children in following and obeying all the rules and regulations of ICWS Al-Andalus programs. I promise to never hold Al-Andalus, Islamic Center of Western Suburbs, nor any of their teachers, staff, or students responsible in any way for any type of physical, financial, emotional, or mental harm of any kind. I also fully accept that the children can be reprimanded, suspended, or expelled at any time, for any reason whatsoever that the administration decides.

I hereby absolve Al-Andalus, Islamic Center of Western Suburbs, and all their affiliates from any responsibility or liability due to accident or injury of my child while realizing that Islamic Center of Western Suburbs will, Inha’Allah, take all reasonable precautions for the safety and wellbeing of the children.

Keeping all the above-mentioned in mind, I would like to request admission of my children as student(s) of Al-Andalus summer camp.

PHOTO RELEASE the Islamic Center of Western Suburbs takes photographs or video of participants for promoting our programs, services, events, activities, and facilities in our brochures, website, or agency social media, etc. By participating in or attending any ICWS activities, the participant (or parent/guardian of a minor participant) agrees to the use and distribution of his or her image (or images of minor child/ward) in photographs, video recordings, and any other electronic reproductions of such activities for any purpose without inspection, compensation, or any other consideration now and in the future.

MEDICAL ASSISTANCE: I give medical consent for treatment and release of information in the event of an emergency at any hospital, urgent care facility, or physician clinic.
